Perform the following to uninstall the app from Jira:
Navigate to Settings > Manage apps.
Locate 'Create on Transition for Jira' using search.
Select the resulting app to view the respective details in an expanded pane.
Click Uninstall. A successful uninstall message is displayed once the app is uninstalled.
