How to create issues higher than Epics in Swanly?

At present, Swanly does not have the capability to support higher hierarchy levels - Epic issue type is the highest level you can set in Swanly to see things in the hierarchical view.

If you add a higher hierarchy level in your Jira project (Initiative, for example) - Swanly will still recognize Initiatives as issues but it will not recognize them in a hierarchical view. Basically, you will see them as detached issues on the roadmap.
