Create a page → insert Excerpt macro → insert the Survey and Vote macros within it → publish/save the page.
Create another page → insert Excerpt Include macro to pull the Excerpt data from the source page onto this page.
This workaround is applicable for Confluence Datacenter version.
Note: The same can be achieved with MultiExcerpt app using MultiExcerpt and MultiExcerpt-Include macros. Please note that this app requires a valid license.
