After updating SQL for Confluence (Pro Edition) app to the latest version i.e. 10.4.1, the app gets disabled.

This issue is caused by the last update of Confluence Cloud Migration Assistant app (CCMA) listed here https://jira.atlassian.com/browse/MIG-763
Workaround
Disable the Confluence Cloud Migration Assistant (CCMA) app.
Enable SQL for Confluence app
